Tools to get Veterans onto the Registry and into housing faster, plus how Home Base MN routes people to you.
How the Registry works
A by-name list
MDVA's Homeless Veteran Registry tracks each Veteran experiencing homelessness by name, so partners can coordinate and no one is lost.
Biweekly case conferencing
Meetings are held by region (Hennepin, Ramsey, SMAC, and Greater Minnesota) and review every Veteran active on the Registry.
Participants include MDVA, the U.S. VA, county social workers, CVSOs, MACV, shelter frontline staff, and other community partners.
Refer a Veteran
- Get the Veteran's consent. Joining is voluntary, and declining doesn't affect eligibility for services.
- Complete the Statewide Release Form (PDF) with the Veteran, and submit it as MDVA directs. VERIFY submission method

How Home Base MN works with you
Warm handoffs
Each Veteran request goes to one agreed partner, based on region and biggest need, with the Veteran's consent.
Your response times
We show Veterans only the response times partners have committed to in writing.
You review your content
Each partner approves the pages about its programs before launch, then every quarter.
